Policies & Consent Form

POLICIES & CONSENT FORM


In [business name]’s commitment to provide an outstanding and professional experience to all of our clients and out of consideration for our time, we have adopted the following policies:


ARRIVAL TO YOUR SESSION


Please be ready for your appointment(s) [10 minutes prior] to the scheduled starting time so your animal is clean, ready and relaxed. The person who rides the horse will need to be present and ready to ride during the appointment. Riding will ideally take place in an arena with good level footing. 


Please have your horse secured in a location of the barn that is quiet and comfortable.  For a more relaxed experience for your horse, avoid areas of high traffic and commotion (ie. main barn aisle during feed time). 


LATE ARRIVAL POLICY


Late arrivals made [15 minutes] after scheduled start time will be automatically cancelled and charged according to our cancellation policy.


At the discretion of [business name], we may be able to accommodate a partial or full appointment for late arrivals.  To be fair and courteous to everyone’s time, your service(s) may be shortened in order to keep on schedule.


CANCELLATION POLICY


At [business name], any appointments not canceled within [24 hours], no-show appointments, OR if you are more than [15 minutes] late for your scheduled time are subject to our cancellation policy fees.  Multiple services (two or more) require [48 hours] cancellation notice. 


Cancellation fees are as follows:

     - Cancellation fee is [$100]

     - No-show fee is [$140]

     - Rescheduling appointment within 24 hour period is [$120] ([$XX] will be refunded at next appointment)


We value our time as well as yours, and cancellations or no-shows do cost us money. We are not only here to help your animals feel better but to make a living as well.


​We understand that unanticipated events occur in everyone's life. Cancellation policies may be waived at the discretion of [business name].


TRAVEL


Travel charge is calculated as follows:

     - [$60] for [100 radius]

     - [$50 for 2 horses]


Travel charge [may be split among clients within the same barn, and is waived after 4 horses.]


PAYMENT


Any deposits paid to reserve an appointment will be deducted from the final amount.  During the appointment, [name/business name] may suggest additional services not included in the original estimated price. The owner reserves the right to decline additional services that may change the final price, but understands in doing so they forfeit potential benefits from those services.


All payments must be paid to [name/business name] in full after services are rendered, and may be paid via [cash, check, credit card, Venmo].  At this time, [business name] does not accept forms of payment through [CashApp, Apple Pay].


REFUND POLICY


We do not offer refunds on [deposits made to reserve a service, payments for services rendered, or cancellations received less than 24 hours before a scheduled appointment]. 


CONFIDENTIALITY AND CONVERSATION


The discussion between [business name] and the client is private and confidential.  We are happy to share our professional expertise, but reserve the right to decline discussions of a political or private nature. If you would like us to consult with your other horse professionals we are happy to do so with your consent. 


LIABILITY DISCLAIMER


Working with any animal involves inherent risk. The owner shall agree to not hold [business] responsible for any injury or condition that may occur to their animal. [Business] agrees not to hold the owner responsible for any injury that may occur to her during the [appointment]  (e.g. from a bite or kick).


[Name with Business name] reserves the right to stop an appointment at any time if they feel their or the horse’s safety is in question. {Name with Business name] also will not fit a saddle to a horse that is in poor body condition, or obviously lame, or ill where riding would be considered poor horsemanship. 


EXISTING AND NEW MEDICAL CONDITIONS


It is the responsibility of the client to keep [business name] informed of any medical treatment, current medications, or changes in the animal’s health condition. If necessary, the client may be asked to obtain written permission from the veterinarian, chiropractor, physical therapist, or other health professionals that our services are safe to continue. 


VETERINARY DISCLAIMER 


If your horse is recovering from an illness or injury, or in a condition that warrants continued medical treatment, [business name] may require clearance from a veterinarian prior to seeing the horse under tack/being ridden. The owner is advised to consult a veterinarian prior to the animal being fitted with tack,  with any medical concerns or questions and/or to verify with the veterinarian that putting the horse in work is appropriate for the specific animal in question.


Lauren is a SMS Apprentice Saddler and A Saddle Fitter NOT a veterinarian, and therefore is not able to answer any medical questions nor diagnose or treat any illness or disease.


COVID DISCLAIMER


Our staff is part of the vulnerable population that can be dangerously impacted by COVID and other communicable illnesses. By attending your appointment, you agree that you are not currently experiencing any of these symptoms:

     - Cough

     - Shortness of breath or difficulty breathing

     - Fever

     - Chills

     - Muscle pain

     - Sore throat

     - New loss of taste or smell

*Please note: Other less common symptoms have been reported, including gastrointestinal symptoms like nausea, vomiting, or diarrhea.*


You agree that you have not:

     - Tested positive for COVID-19

     - Knowingly been exposed to someone with COVID-19

     - Recently traveled to an area with a high infection rate

     - Been in an area where social distancing was not properly observed

     - Been to a nursing home

*If you have experienced any of the above, please reschedule your appointment at least 14 days from now.*


CONSENT & RELEASE


I understand that since there are many variables, it is difficult to predict the duration and frequency required for the recommended saddle fitting procedures, and there is no guarantee that we will be able to make a saddle or other tack fit on the day of the services provided by [business name].


I agree and consent to assessment of my equine.  I understand that my equine’s personal and health information will be collected.  I understand that all information that I provide will be kept confidential unless required by law.  I understand and consent that my equine’s medical information may be shared by the various care providers involved in my equine’s care and treatment.


My horse is not under the care of a veterinarian for any reason that would contraindicate riding or work.  I understand it is my responsibility to inform [business name] of any changes that would make riding/working contraindicated for my horse.


I HAVE READ THE ABOVE AND AGREE TO ALL POLICIES AND RELEASE [BUSINESS NAME] FROM ALL LIABILITIES.
